How can I edit the Title Tab of my form?

I used a ready made template (LeadUs Mentoring Program Application Form) which I edited but I can't change the Title Tab (as seen on the attached photo) to the title I want. Please help me on how I can change it. Thank you!

You can navigate to Settings > Form Settings > Show More Options > Page Title. Please refer to the attached photo for your reference.

Thanks for your fast response but it didn't solve my problem. It's for renaming the file name not the Title Page. Anyway, I did change it. See attached.

Please note that the value you set on the Title Page will be shown in the browser's tab title. I can see that the title of your form is now already changed to a different title.
