Visual Report will only pull in one question from the form

We are trying to add a visual report and the report will only pull in one question.  I've tried to add a visual report to other forms as well to test and make sure it is not just a problem with that form.  Our other forms are having trouble too.  Is there a known issue with the visual report builder?

Hello, we were trying to create pie charts per question. We only had one option for the visual report and that is what you created. This is not much different than the excel export.
Maybe we are not able to due to the style of form we used. We used the card format.

We apologize for inconvenience. I also test the Visual Report using a clone of your form, but so far on my test, the Visual Report was working as intended. 

I was able create pie chart for each of the questions/fields from the form. You can check the test report using the following link

In my test Visual Report you can see that each question have pie chart assigned.

Can you please clarify if this is not what you were trying to add on the form.
