I have several forms that we have put on our website for our patients to fill out and you notify us so we can print them and put in their charts. Why does the Financial Policy Forms and Assignment of Benefits forms only have signed signatures know form with it. We are also noticing that not all forms are coming out the same, like if they skip a question then it doesnt print the question. I really don't like having to email questions and would really like for someone to call me asap.

Please note that you can modify each PDF file and Notification email. You can access the PDF from the Notification email:
By default, empty fields will not show in the PDF file. You can change this from the PDF editor. You first need to open Layout Settings:
From there, open the Customize tab, and disable Hide empty form fields:
