My form doesn't load and cannot fill text box

Hi, I try to use this form yesterday and cannot use it. In Chrome and Firefox stuck loading page.

I cloned your form and I was able to replicate the issue. I suspect that is due to the DatePicker widget on the first page. I tried deleting the Date Picker widget and it fixed the issue. Are you using that widget? If not, I suggest deleting the widget too.
