Why is my account suspended?

    The form that I created is only a reservation form for our website.

    Upon checking, I've found that you're offering a service using an account registered with a free domain email, therefore in order to reactivate your suspended account, we will have to delete the offending form Mortgage Pre-Qualification Form.

    Or, kindly provide us with the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting suspended again. Send these proofs to reviewers@jotform.com with this thread number, which is 2412936, as your subject.
