The title appears twice on the preview

https://form.jotform.com/201768049927062   When you look at form in the "building" stage, there is only one header and subheader.  Yet when you observed it in "preview" stage, the header and subheader appears twice.  How do I get rid of the second heading?

I've cloned your form and it seems that the header has been added twice. You can simply delete the extra header field from your form.
