    <script type="text/javascript" src="https://form.jotformeu.com/jsform/73404916968367"></script>

The form belongs to a paid account. Is that your account? If yes, let us know so we can ask you for more questions.

If that's not your account, you can create a JotForm account and clone the form then you can embed the form to your website.

We'll wait for your response. Thank you.

    Hi VincentJay,

Thanks for replying to my question. This account was created by the webmaster we hired to build our website (www.duvivieretassocies.fr) so i'd have to assume it is? Unless he has an account for multiple websites he created. Can you at least tell me what email address the messages sent through this form are sent to? The problem is, we had several clients telling us we never replied to the forms they sent us on our website, for a good reason because we never got them.

You're telling me it's a paid account, you probably can't tell me who actually pays the bill but maybe you can tell me if it's my company? (Duvivier & Associés). The webmaster's company on the other hand is supposed to be called GIH multimedia.

Thanks a lot for your help, we'd really like to recover the forms they were sent a we never got.

You can contact the form owner (aviard@gih-multimedia.com).

You can ask the form owner (aviard@gih-multimedia.com) to move the forms to your new JotForm account. In this way, you can manage the submissions to the form.

If the credit card use belongs to you, we can investigate it further and possibly switch the form ownership.
