Enabling of my Jotform Account

    Hello!

    I need you to please enable my account once again.  We need to use your forms for our upcoming school enrollment.  I have received an email that my account has been disabled.  And i don't think i have made violations to our agreement.  If i made one, kindly please notify me so i can make the necessary corrections.

    We are really crunched for time and we badly need to be able to gain access of the forms that we have already built up.

    Hello! Your form was suspended as you're collecting banking details from your users in your form https://www.jotform.com/form/201779226112451 (screenshot attached below of the offending fields)

    Please note that collecting bank account details from general public isn't allowed in Jotform as stated in our Terms.

    Phishing. You agree that Jotform Inc. may terminate your Jotform Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    If you're not collecting this information from general public but from your own clients, then you need to install two required widgets on your form:
    1. Add the Terms & Conditions widget (or the Short Scrollable Terms widget) for your users to read and agree.
    2. Add the Signature widget so your users can sign and agree to provide such sensitive information.

    I have removed the offending fields from your form and have also re-activated your account, you should be able to access your account now. You can re-install these fields after installing the mandatory widgets in your form to avoid further suspension.
