I created a from from a PDF on your site. First, I want to say very easy to use and appears to be exactly what I am seeking for my plastic surgery practice clients.
I created a form from a PDF entitled Informed-Consent - Reduction Mammaplasty. I previewed, filled in and downloaded the completed form. The download did not include some fields I added, such as signature and included some elements from the initially uploaded PDF that I had deleted while editing on Jotform. Why did it do this?
Also, I like the signature field, but it would be better if I could choose to type the signature into that field and it types it in cursive. Docusign does this. Is there an option something like this?
Finally, is there a more secure way for the patient to fill in the information or send the the information where the medical practice can receive it?
