    my entire folder was accedentally erased

    I am not sure if I am understanding your inquiry correctly. Could you please provide more information regarding the folder you are referring to?

    If you accidentally removed any of those folders, you can create it again and move corresponding forms there:

    Please note that deleting folders won't erase forms that were in it.

    I've also checked your account's activity log and you have recently deleted this form but it's now restored and active:
