I am connecting my One Loudoun General Contact Form to my Mail Chimp account. The API key is all set and the import goes OK - but the individual fields aren't displaying when I try to match up the fields using the MailChimp Integration Wizard. After matching the first field, the rest of the fields disappear from the drop down and are not available to link.

Make sure to have the correct fields in your mailchimp. Full Name, Email and Phone fields can be matched using normal text box on mailchimp. However, you have to use address field on mailchimp for the address field in your form.
I cloned your form here and created these fields on my MailChimp Form, I can easily matched them on the integration wizard.
Kindly make sure to have the correct field type so you can integrate them. Try to re-integrate your form once more. Thanks!
