Error Message Following Submission

Hello,

We are experiencing an error message  that is followed after we try to make a submission using the form. Could you tell me what might be causing this issue and how we can fix it?

The message says "Oops! Error on query: Error returned: MySQL server has gone away"

We do apologize for the inconvenience.

It seems we had an issue with our data server however was fixed by our developers.

Kindly test your form again and let us know if the issue still remains.
