How to use the Show or hide condition

When using the show/hide field.

If: Do you have a fever

State: Is equal to

Value: Yes

Do: Show

Field: This is where I get stuck...… I cannot free type in there and the choices do not apply. I need it to say, "Please call the office to reschedule your appointment."

Please advise what I'm doing wrong.
