Can you tell me how I can fill out a form that was sent to my email address?

Did you mean a form that is embedded in an email message? Or, it's just a form URL? I ain't sure if you can fill out a form when it is embedded in an email message. Most common email services does not allow script tags in the message area, our form has it so it may be impossible for you to fill it out. Form URL will do.

don't know what a URL form is!
The form I was sent was embedded in the email and I guess that's why I
can't access it.
The URL of the form is one like this:https://www.jotform.com/form/31837218653862
It's the same that appears in the direction bar of your browser (Chrome, Mozilla Firefox, I Explorer).
So, what my colleague asked you is if the form that was sent to you is the URL as the one I give you above or is the form itself like this:
If the one you received was the URL you can click it and fill it out and send it by clicking on Submit.
But if not, as my colleague said it may be impossible to fill it out.
