Customer qualifying form

    Hello JotForm Support,

    I'd like to create a customer qualifying form where the user would select the answers to company specific questions. If they select an answer that qualifys then, the form would let them continue. If not, the form would display a message that they are not qualifyed to be a customer. Is it possible to create this type of form? Please provide instrucitons.

    Hi Vic!

    Yes that is possible using Page Breaks found under Survey Tools in the Form Builder. Basically, you can separate the questions, and then have part of the page be a text that indicates that they are not qualified.

    Then click on Setup & Embed, then Conditions. Set a condition using the Skip to a page or end of the form condition. The condition should be something like: If question A is equals to "whatever answer", then skip to a particular page (a page with more questions or a page indicating they are not qualified)

    In your form I can see you have a radio button for "I'm not a customer yet" and "I'm already a customer". Perhaps if you could provide us more details with what you are trying to accomplish, we can help you with that. Thanks!
