How to create a Medical History Form that is HIPAA compliant ?

How can I create a Medical History Form that is HIPPA compliant using both the foundations of one of your offered templates which are fillable in PDF form and adding additional questions unique to the purpose of making and offering the form to be filled out ?

I can see that you are upgraded to the silver plan and you already have HIPAA enabled on your account. Your forms should be HIPAA compliant when you create them.

Unfortunately, fillable PDF forms are not HIPAA compliant. The feature is not available for HIPAA accounts.
