Can I change what shows up when I link this to my website for members to fill in? (i.e. can the web info/form number be hidden and instead say something else like "NorwoodJWCMembershipForm"?

You can create a custom url for your form. Please follow the instructions below while on the Edit Form to do so.

If I understand your requirement, you don't want to show the form url to your users correct?
If you embed forms in your website using any of the embed script, your users will not be able to see the form url unless they go to view source of your webpage. Please refer this guide: http://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use
If you want to use your form's direct link with your own domain, you may consider using custom domain. Please refer this guide: http://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform FYI, we do not support SSL for custom domains.
