My account has been suspended?

    Hello Jotform,

    Please could you tell me why my account has been suspended?

    I recently tried to create a payment methods form (which we have on paper but wanted to be transferred digitally) and I think because I had fields asking for credit card information, you have viewed this as phishing?

    Please can you reinstate my account immediately - I used jotform on a daily basiss for my company:

    I have two emails associated with my account ( and as I got married and have a new email address to reflect the name change.
