    Check box with comments

    I have a very simple task but for some reason can't figure out how to customize the form to allow it. 

    I have:

    Question 1?

    Yes _

    No _

    Comments _________________


    The only way I can figure to do this is to use two questions and it takes up a lot of room and I have to deal with comments that don't really look like they are part of question 1. 

    OR - I can enable your "Allow other" featue but is there anyway to remove the default text and checkbox that is in front of "Other"

    Hi, Did you tried Free text HTML for this?

    Add a Checkbox field and then add a Free Text HTML Field and put your comments there.

    Do you want text box in comments area or its just a plain text?

    To Reduce the Space between the checkbox and the Free Text HTML  you can inject this following CSS code

    #text_3{ margin-top:-30px; }

    Where text_3 is the ID of my Free Text Field.

    I was looking for a text box where the user can submit their comments as part of each question. There are going to be a lot of them, ideally we wouldn't even use a new line for each one so it would look like.
