Unable to integrate with Salesforce.

Hi,

I have a form called 'SALESFORCE - Clone of **** intake form' (it's a copy of the actual form so safe to play with). It has been setup to integrate with Salesforce but contact records are not getting created in Salesforce. The Salesforce access credentials are correct as I'm able to integrate a simple 'Test form' correctly. Can you find why the integration is failing?

I am unable to check your integration since your account is HIPAA compliant, but I have cloned your form and I was able to integrate your form's clone with Salesforce.

Could you try integrating one field only within your form and checking if the integration works?
