Hazard Assessment Form - Q Construction Management: Permission

The form just gives me the same message, that I don’t have permission. Then it asks if I want to request permission, which it did last time. How do I fix this?

Hello Frank,

As advised by my colleague on your previous ticket, you can change the form's settings to Public on the Publish page and this will remove the request permission to access your form:
