Why are my forms showing "disabled"?

    I was in the process of editing a form when I received the notification that my form was disabled. I refreshed the page and tried to access other forms but one again received the disabled prompt.

    As checked, your account has been auto-suspended by our system as this form: MCLS Library Card Registration contains plain text fields that collect Password/Login credentials. It is considered as phishing and violates our Terms of Use

    You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    I have deleted the said field and reactivated the account. You can add it back to the form but use a different term. Instead of using  "Password", use "Access Code" or any other terms you prefer.
