I have noticed that when I make 3 columns of checkboxes in word, then convert it to a PDF, and finally upload to smart online form through jotform that it takes data that are side by side and puts it into the form builder as one long thing.
I want it to break up Meats, grains, and Vegetables and put the appropriate data points already in the right spot.
I also tried to click and drag the check boxes under the correct topics but was unable to do so.
Need help to make it easy for me to re-upload anytime I have changes to make.

You can position the form fields side by side by following this guide: https://www.jotform.com/help/328-How-to-Position-Fields-in-JotForm
I recommend increasing the form width in the Form Designer page and then use the Shrink feature to each form field.

As you can see. Even when I increased the width of th eform and use the skrink feature it does nothing. It still does not space out all answers to the appropriate underlined topic.
As you can see the original PDF looks nothing like the smart PDF.
Any more ideas. I know I can manually do it. But I do not want to have to if there is another way.
