I see the notification email is incomplete, is there an easy way to rebuild it? or to get the default email notification?

Hello Carolyn! A form can have multiple email notifications. So, you can create a new email notification for your Form.
Whenever a new notification is created, it loads all the defaults including the email content (that would include all of your form fields).
You can delete your current email notification with which you're facing issues.
