I'm unable to map my Zoho CRM fields to my JotForm fields

I'm trying to map my zoho fields to the jotform fields. all I'm missing is the last name and email and I keep getting an error. I don't see anything when I choose Select From Field. I don't know what the problem is. 

Is this the form in question https://www.jotform.com/202395981702055? Make sure to add fields in your form first so you can pick them on the dropdown field.

You should have an Email field and a Short Text field in your form so you can map the two fields shown in your screenshot.
