We are truly sorry about that. I have now unsuspended your account after finding no violations of our Terms. Your form should now be back online and work normally as expected.
To avoid this from reoccurring I recommend you name all your field to avoid having them suspected to be used of phishing.

Hi, my account has been suspended and i dont know why it says because of phishing but i dont ask people for passwords and account information? why is that

Upon checking your account I found that it's suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.
Upon taking a closer look at your form, I can confirm that it's not being used for any phishing activities. I have activated your account now.
