Does the patient have to create a JotForm account to fill the form out?

The email that the office staff will receive has all information hidden! How can I make this info appear?

Does the office staff need to login to JotForm to get the Submissions?

    Thank you for reaching support,

Does the patient have to create a JotForm account to fill the form out?

    > Not necessarily, if the form has no access restrictions, anybody can fill out the form.

The email that the office staff will receive has all information hidden! How can I make this info appear?

    > I check the account associated with email address: vreitz@cgicompany.com and it is under our Corona Responders Program making it HIPAA Compliant, along with its data protection. Only the form/account owner can access the submission data.

Does the office staff need to login to JotForm to get the Submissions?

    > Yes, but they need to access the Account where the filled out form was created from in order to view the submissions.

Let us know if you have any further questions.

    Best.
