How can I create a form for internal employees to request a form build?

Hi,

I'd like to create a form where our internal employees can request new forms or existing form updates.

1. How can I hyperlink to a Form Request Template document for the user to download within the form?
2. Do you have any other clients who have successfully created a Form Request form? If so, any helpful takeaways?

Hi there,

I'm quite not sure what you want to accomplish. Can you provide us more details? Do you mean to attach a downloadable document in the form? If so, that can actually be done by using a paragraph field and inserting the link to a word/phrase to make it more presentable.
