I need to be able to have customers use their smart phones (iPhone, blackberry and android), to fill out this form. Is it possible? I don't see a non-java based version any where that I can use and I know that iPhone doesn't support java.

So far, we haven't had any problems displaying forms in the leading mobile platforms available. Can you specifically tell us what issues your users are experiencing with your forms?

I uploaded the form onto our page and see no results on my blackberry
device. I have to test several devices. But can't even see it on mine yet
so I supposed it wasn't working. The image you sent didn't come through.
Could it be the way it's imbedded on the page that is not allowing me to

I see what your problem is now as I was able to replicate the issue on my own mobile phone. It looks like the problem lies with the height of the form. Since it is embedded on a block that doesn't have a specific height, the form is having problems with expanding the height of the block.
Solution is to use iframe embed codes instead of script embed <>
To get your iframe embed codes
1. Go to Setup & Embed tab
2. Click Embed Form
3. Click iFrame
4. Copy the codes provided in the next screen
