    Hi Metropet,

Can you please verify the name of the form that you used on your website? I cannot see it in your 6 existing forms under metropet username? Please verify that the form was created using Jotform Because it says there that it was generated by PhpFormGenerator.

Looking forward to hear from you.

    Thanks,

    Jed

    Hi metropet,

To address this one what platform do you use? ex. wordpress,Joomla, drupal etc.. I can see that some of your forms is working live already like this one http://www.metropetmag.com/about-mp/contact/127. What you can do is to embed the form to your website, you may want to take a look at this instruction on which embed code to use http://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use

    Thanks,

    Jed
