I have seen this done, where a unique one-time code was given to a family to vote.
How can I accomplish this?I have cloned the prior form, but do not know how to accomplish a similar effect.

Hi there - Unfortunately, it's currently not possible to have a unique one-time code for your users before they can fill out your forms. You may want to consider setting a password for your form instead.

Hi again, upon further testing, it looks like you can achieve something similar by having a Short Text field for the unique code and then show/hide the next questions based on the code given. Please refer below:
