Date Field is not on PDF

Hello

Our form "COBRA Renewal Questionnaire" has two date pick fields labeled Beginning date of Open Enrollment and End date of Open Enrollment - these fields have stopped being shown on the generated PDF. Why?

Please ensure that your PDF has all of the correct fields and to Preview PDF. Then, when attaching the PDF to your notification emails, please ensure that you have the correct document.
