Subscription expired

    Good morning,


    We do not know what confusion is going on with our subscription. Yesterday I requested that our account page was updated to reflect the yearly Silver payment that was received by you on 06/07/2020.

    What is the reason that the account is now showing "FREE" and the alerts of submissions, space, form view are asking us to UPGRADE NOW?

    Please correct this ASAP. We are currently depending on JOTFORMS to receive all the Health History forms for new students.
