I added the check box function to my form. It automatically comes up with three check boxes that say "Option 1, Option 2, and Option 3" next to them. I deleted the second and third, and deleted the text next to the first box. It initially saves that way but then it always reverts back to the default with the three boxes with the "Options" next to them. Is there any way to fix this?

That's happening because you delete all the options, and there must be at least one. When deleting the "Option 1" text is the same as deleting the whole option along the checkbox and that makes the fields reappear. So, what you can do is the following:
1. Delete Options 2 and 3.
2. Don't delete the "Option 1" text.
3. Insert this CSS code to your form:
That will hide the text "Option 1". Please refer this guide to know How to Inject Custom CSS Codes. Finally, save your form.
And that's it. Double check if everything is ok or let us know if the issue still persists.
