How to Create a Cart like function with Product List?

Is using the product list feature the only way that I can create a "Cart" of items that will calculate a total to be charged at the end of this form? I am creating a form that will charge the person completing the form based on their selections but I need some selections to force items totheir cart without the person selecting it. For example.

If the person would like to bill their insurance I need it to add two specific items to the cart. If they want to pay cash, two different items should be added. I don't want to the person to get to choose the items because they will likely not select the correct items.
