My form is not working

    Dear JotForm support: I am having trouble getting my form called "Ticket Order Form" working. When I try to preview it, it says "This form is currently unavailable!". Could you have a look and let me know if there is something I am doing wrong? Thanks in advance. Paul

    I found out that the form is disabled that is why it is not working. Please take a look at the screenshot. I have enabled it again so it should work by now.
