My account is suspended. Help!

    Our company's account is suspended. Kindly assist as we get our orders from here.

    We are sorry for the inconvenience this may have caused. As we check your account, it seems to be suspended due to the forms using copyrighted logos. Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting ID numbers and displaying copyrighted logos without registering the account with the company email address is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #2654704 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
