Our Online Application JotForm is not working properly

    We use JotForm for our online application on our website. The form has disappeared and we are unable to have applicants apply. Can you please help? I need fixed as soon as possible.

    It looks like the form owner's account was suspended on October 20th because of spam detection.

    Form owner's username is "jmerch". Do you have access to this account?

    If that is not the case, I cannot move this form to your account. However, I suggest that you clone their form and embed it again on your website with an iFrame.
