review a form

I recently came across a house for rent, and the person sent me a jotform link to submit an application for the unit. But i am unsure if the link is trustworthy. With sensitive information.

I have checked the form and it seems to be collecting SSN and ID but lacks some proper widgets to collect such information.

I have suspended the account and disable the form for the time being while we review further the form.
