PDF on a link

On the below link, fillable PDF that would save the info once typed in doesn't anymore. Please advise

I am not sure if I have understood your issue correctly. I did check your form and it seems you have embedded fillable PDF in your form.

Is it a fillable PDF built using JotForm PDF editor? If yes, please note that fillable PDF can be submitted using Adobe Acrobat Reader or Foxit. I also checked the embedded PDF and it does not have any submit button as well.

Are you expecting that when the form is accessed by user, they can also input in the fillable PDF along with the other fields of form and when the form is submitted, the data of PDF also is captured in submission?

Unfortunately it is not possible to capture the data of embedded PDF in form's submission. You can add a link in form for users to download the fillable PDF, fill up the fields and then upload it in form using file upload field.
