Hello, I've gone ahead and cloned your form, do try my version of the form:

    http://support.jotform.com/form/32517336983865

I've gone ahead and added additional fields like so:

    USER1

    USER1
    USER2

    USER1
    USER2
    USER3,. etc.

This is how I've also left the conditions:

Using unique fields, never repeating a user field on a condition.

    1

Please clone and try out this form.

If you need additional assistance, do let us know. Thank you.
