How to insert a form as an option in a multiple choice?

And if I understand correctly you would like to have the last option of the Multiple-choice field linked to another form, am I correct?

Unfortunately, this is not possible inside the Multiple choice properties, but you link another form by adding the URL inside your form but using the text tool

Thank you, but if I want to click on Additional Notes for example, and have another four options pop up because I clicked on Additional Notes, how do I do that?

You may use our iFrame Embed Widget and place it inside a Paragraph field, use a condition to show the field.
