hi! i am able to place my url link in by website but when i click on the patient form it does not allow me to input information and submit. it appears to only be a pdf. is this not an option anymore because of hippa. I want the patient to be able to click on the link, input their information and submit from our URL/website. Right now I am having to send them the link from my email to have the fill it out, which is not efficient. please advise. I might be missing something. thanks.

I did check your account "PinkLink2301" and you have not enabled HIPAA compliance. Please note that when you enable your account to HIPAA compliance, the submit button on the fillable PDF is not supported.

Coming back to your issue, I would suggest you to please share your web page URL where you have added form link, and we will take a look. Please note that fillable PDF form can only be submitted using Adobe Acrobat Reader or Foxit. Fillable PDF cannot be submitted from browser.

We will wait for your response.

We have the silver plan which is HIPAA compliant. It is listed in our account information. Is there something outstanding?

Will a patient not be able to fill out and submit the form from our website - link - below. I need to know how to set it up so that it works this way, or have an alternative way to enter the information.

    Thanks for your help.

this is very odd...when I click on the link i can open the female patient form and it looks as though the information can be input and submitted? This was not happening previously. If this is working now then I need to confirm the form is HIPAA compliant. Please advise.

The forms on your website are working fine, but they are not enabled for HIPAA as your account is not enabled for HIPAA yet.

Please note that upgrading the account to a silver plan does not automatically enable HIPAA features. You have to manually enable HIPAA compliance features after that as explained here:

I signed and submitted the BAA yesterday and have a copy of it form jotform. I already did that. This is trying to make me sign it again. I am confused. What is going on? I've already done that. What is the next step? Thanks.

I just went through the BAA process again and received a copy saying I am not HIPAA compliant. When will it show on my account? Is there anything else I need to do? Please advise. Thanks.

    Hello,

HIPAA compliance is now enabled on your account.

Now all forms in your account have the HIPAA compliance features we offer.
