To overcome this problem, it's probably best to embed the form via the iFrame option (Setup & Embed > Embed Form > iFrame). After clicking the iFrame button (highlighted below), copy the code provided by the wizard and substitute it for the current form code on your page. However, before doing so, please read on.

Since your current form only contains test submissions, I suggest deleting it and cloning this form to use in its place. Be sure to change its title. Other wise notifications in your email from this form will have the subject New Submission: Abajan's File Upload Form :)
If you require further assistance with this or anything else related to our forms, please don't hesitate to ask. Our team will be glad to help you as best as we can.

Sorry about that. I realized that the image didn't link to anything and made the adjustment, probably soon after you viewed the reply. Try it now.

The following response from aviadstack showed up in my inbox:
thanks - let me ask you this... why do you think embedding the html is giving me a hard time. i want the form to look a specific way and to have the upload status too. it kind of sucks that i would have to use an iframe and that i don't have complete control over the source code. is there a way to fix this problem? it would be great if in the future i could also choose to use my own image rather than have to use the default orange one. and one last thing... is it possible to remove the "no file selected" if i am using the generic solution?

Embedding via an iFrame usually clears up certain problems like users being able to submit forms without required fields being completed. So, I figured using that method would resolve your situation as well. Presently, as you alluded to, it's not possible to replace the big orange multi-upload button with an image (as can be done with the submit button) but I'm pretty sure our developers are working on something.
That's about as much as I can tell you at the moment but believe other members of our team will be able to offer suggestions/workarounds when they read this thread.
Incidentally, the upload status per file is shown when using the multi-upload button.
