I would like to embed this code on my form for a submit button to take me to a different payment page. How do I do this?

I know that Jotform has payment integration with Authorize.net but does not offer the ability to use a check instead of credit card. That is why I need this new button.
In order to add this custom submit button on your website and direct your user to simplecheckout authorize.net link. Kindly double check the LinkId value. Because I have tested it on my form I am getting an error. "The item you requested is no longer available."
Here's the instruction on how to put the custom submit button on your form.
1. Under Power Tools on the left panel of your JotForm editor, there is a Free Text (HTML) field.
2. Drag that field to your form.
Once you have added that field to your form. Click Edit HTML
Then, Empty the body of the text and click HTML button below.
Then the HTML editor will pop-out. Empty the texts and paste the code you have provided there.
Then click UPDATE.
Once you are done, you will see the button on your form.
Click preview and try an actual submission on your form and you will be directed to the checkout page.
However, As I have mentioned, you need to fix your code to be able to work properly.
Also, you need to remove the default submit button on your form and leave the one you have added to avoid to see two submit button on your form.
