    Hi vonnie52

Assuming that you wish to replace the form currently at http://www.goodsamaritanhhc.com/contact_us.html with your JotForm "Contact Us" form whose URL is https://www.jotform.com/11284038018, do this:

    1. Load your JotForm "Contact Us" form in the form builder and click the "Setup & Embed" toolbar button

    2. Click iFrame (highlighted below) and copy the code provided to your clipboard (Ctrl + C)

    3. Open your "Good Samaritan - Home Health Care" page for editing and strip the following portion of code from the source:

    4. In its place, paste (Ctrl + V) what was copied to the clipboard in step 2 and save your changes

Be sure to let us know if this solution fits your requirements and if we may assist you with anything else related to our forms.

    Thanks.

P.S. If your form doesn't look right, clone and embed this one instead.
