    Not recieving notification email.

    My form is not being sent to my email - I have changed my email address and still it does not come through. I need this info.

    Hello chaplainmark,

    This form (#31634626395156) you have mentioned above is not in your account. 

    I checked all forms in your account and could not find any problem. There is not submission records in any of your form so could not verify but the last successful notification email was sent on Sep 13, 2013. Here is the log:

    Can you please confirm if account with username "markoreilly" is also your account. I see someone logged in in both the account from same IP address.

    If yes, Upon checking I found that you have not selected the "Sender E-mail". That seems to be the reason of you not receiving your submission emails.

    You should select/add the "Sender E-mail" in your notification as this is required to be added for your notification alert to work. You should also input your email address in "Recipient E-mail" field.

    You can select "noreply@jotform.com", "noreply@formresponse.com" or you can also setup a custom "Sender E-mail" option. This guide should help you:
