Valley Community Church (Weldon, NC) has an account, but the person who set it up can't remember the user name and password. He has a personal account under his personal email, but he cannot get to the forms we are using on our website.
The only thing we can tell you is that when the forms are completed, they are emailed to me firstname.lastname@example.org. We are at the limit and our account has been disabled. We need our account info so we can sign up for the next 3 months.

I found two guest accounts under the email address email@example.com, one without a form and the other with one form that is over the limit of 20 submissions for guest accounts.
You must register for an account so that you will be able to log in with a username and password whenever you want to access your forms. You can also upgrade to Premium or Professional to enjoy a higher submissions limit.
Also, you would need to import those forms onto your newly registered account.
