Abuse Department for ,
Synacor is a mailbox/internet service provider. Our website is http://synacor.com. Our clients are MSOs, Telcos and independent ISPs. My primary responsibility is to drive/maintain anti-spam efforts, best practices, and anti-abuse policy enforcement.
It came to our attention a few moments ago that you might be hosting a phish URL which is having an impact on our clients that we host and manage email for. This phish URL is being used for illegal, illicit activity to obtain account credentials from our client's end-users.
* Phish URL: http://form.jotform.us/form/32622717161146
* Phish snapshot (attached), filename "Bad"
In good faith, and supporting the current fight against spam & cybercrime on the internet, I request for you to have the phish URL - and probably the entire webspace under the offending domain - suspended immediately. I'd appreciate a reasonable explanation, detailing your findings and the actions you have taken against this phish URL.
Please treat this with the highest amount of importance/severity level you can, as this is a risk to innocent parties across the Internet.
Thank you for your cooperation in this matter.

Thanks for reporting it to us. I have suspended this form and user account so with the IP Address. If you see the same form again, please do not hesitate to contact or report them to us. We have an automatic phishing detector in our server. However, they uses their knowledge to manipulate the form html code that is why they are able to bypasses the filter.
Thanks for your time and cooperation.
