How Can I Track Anonymous Evaluations?

    Hi, I'm loving JotForm so far, there are SO many possibilities! I run a non-profit therapy clinic where the therapists are students in training. They take 3-4 classes per quarter and need to evaluate them anonymously.

    1) How can I be sure that each student completes the evaluations WITHOUT having their name attached to the actual evaluation (so their feedback is anonymous)

    2) Since they will need 4-5 class evaluations per quarter, what is the best way to send them to them? Is there a way they can see which ones are still incomplete?

    What you can do is add a text field(respondent) and it. You may prefill the respondent field via the direct link.

    A form URL (same form) with the prefilled respondent code (e.g., A, B, C, etc...) will then be sent to the corresponding person.

    e.g.,

    http://www.jotform.com/form/xxxxxxx/?respondent=A

    http://www.jotform.com/form/xxxxxxx/?respondent=B

    http://www.jotform.com/form/xxxxxxx/?respondent=C

    When the form is submitted, you will only see the respondent code and the survey data. As long as you do not have an idea of how the respondent codes are assigned, survey data should remain anonymous to you.

    Do you want them to be able to see all the submissions? or you want to be able to see all the incomplete submissions?
